Definition

IC7Fc represents a novel paradigm in exerkine-based therapeutics: a genetically engineered designer cytokine that combines structural and functional elements of IL-6 (discussed above) with ciliary neurotrophic factor (CNTF) to harness metabolic benefits while circumventing inflammatory complications. PMID 41134879PMID 32563931 CNTF is a 22-kDa neurotrophic cytokine that promotes neuronal survival and regulates energy metabolism through binding to a tripartite receptor complex (CNTFRα/LIFRβ/gp130), activating JAK/STAT, MAPK, and PI3K/AKT signaling pathways. PMID 36065930PMID 7809179 While CNTF demonstrates potent metabolic effects including body weight reduction and improved glucose homeostasis, its clinical development has been limited by side effects and the fact that it is not secreted by muscle in response to exercise. Exerkine classification

The creation of IC7Fc exploits the shared signaling biology of IL-6 and CNTF, both of which signal through the common receptor component gp130 despite using different receptor complexes. PMID 28620096PMID 1617725 The engineering strategy leverages the modular organization of cytokine receptor recognition sites, which permits transfer of binding domains between IL-6 and CNTF despite limited sequence homology. PMID 10207005 By fusing an IL-6/CNTF chimeric cytokine to an Fc domain, IC7Fc achieves extended half-life and improved pharmacokinetics while maintaining metabolic activity. PMID 41134879

Critically, IC7Fc signals exclusively through membrane-bound receptors (classical signaling), bypassing the IL-6 trans-signaling pathway that mediates pro-inflammatory effects in chronic disease. PMID 41134879PMID 32563931 This selective signaling mechanism allows IC7Fc to deliver the beneficial metabolic effects observed with exercise-induced IL-6 elevation—including enhanced energy expenditure, improved glucose homeostasis, and favorable lipid metabolism—without triggering the inflammatory cascades associated with pathological IL-6 elevation. PMID 41134879

Therapeutic potential

In preclinical studies, IC7Fc demonstrated marked reductions in plasma triglycerides and total cholesterol through inhibition of hepatic de novo lipogenesis, increased bile acid synthesis, and decreased apolipoprotein B production, resulting in substantially reduced atherosclerotic lesion formation. PMID 41134879 Additionally, IC7Fc showed beneficial effects on type 2 diabetes progression and other metabolic parameters. PMID 41134879PMID 32563931 These findings establish IC7Fc as proof-of-concept that rational engineering of exerkine biology can yield selective therapeutic agents that recapitulate the beneficial effects of exercise-induced signaling while minimizing risks associated with chronic cytokine administration.
